Prime Care Coordination works to help those with intellectual and/or developmental disabilities find the services and resources they need to maintain their independence and achieve optimal health, wellness, and life goals.

At the heart of this mission is the care manager, who offers Health Home core services such as comprehensive care management, care coordination and health promotion, and individual and family support. In addition to a pay range of $22 to $24.50 per hour, care managers are offered:
